How much does medical weight loss cost?
Medical weight loss costs can vary significantly depending on factors such as insurance coverage, deductibles, out-of-pocket costs, medication choice, and pharmacy preference. Some medical weight loss programs are completely “cash pay” meaning they are not billing their services through insurance, and the patient is responsible for the cost entirely. In some cases, patients may submit receipts for coverage or use HSA/FSA funds to pay. Form Health is unique in that it is covered by most insurance plans, including Medicare. However, for those patients who do not have any insurance coverage for our program, we do offer a self-pay option.
