Do I need to meet Contrave prior authorization criteria? What is that?

Contrave® prior authorization criteria is paperwork requested by your insurance carrier to justify the use of Contrave (naltrexone / bupropion). Some insurance plans will require prior authorization paperwork to be completed in order to cover the cost of Contrave. Your healthcare provider will complete the paperwork for Contrave prior authorization criteria. Unfortunately, this is not a guarantee of insurance coverage for Contrave.
